CHAPTER MEMBERSHIP

All new and renewing members should mail membership forms along with local and national dues to GHC ASA, P. O. Box 2871, Houston, Texas  77252-2871.  We will forward the amount of national dues to the national office.  The expiration date on your membership can be found in the upper right-hand corner of the address label on your monthly newsletter. Renewing members: If you receive a renewal notice directly from the National office, please add local dues (if due) and send both their form and the check to GHC ASA. If you mail the form and national dues directly back to the national office or renew on line be sure to specify Chapter 249 and do not include any local dues. Houston Practioners if you renew online please send only $25 to National Office and send the remaining $40 directly to the Chapter. National Membership includes a four issue subscription of "The Advocate" magazine plus other benefits.
